Job Description
JOB TITLE: Special Kids Network (SKN) System of Care Coordinator
Organizational relationship: The SKN System of Care Coordinator reports directly to the SKN Systems of Care Program Project Director.
JOB DEFINITION: The SKN System of Care Coordinator has primary responsibility for supporting the development and implementation of community, county and statewide services to assist families, children and youth with special health care needs (CYSHCN). This includes service system integration, organizational development, service capacity development, and family support services related to achieving healthy outcomes for children, families and communities. This position supports the coordination of family/child-serving systems such as pediatric and adolescent health service providers, public/private human service agencies, schools and other service providers. The System of Care Coordinator will be responsible for creating cross program collaborations among all projects under the direction of the System of Care Project Director.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Project Development and Planning:
- Work with System of Care (SOC) team members to develop project action plans.
- Work with community partners to research, plan, develop and enhance programs for CYSHCN.
- Conduct ongoing research of model programs and projects in related areas.
- Project Implementation:
- Coordinate program activities for statewide initiatives as assigned, including implementation of trainings, workshops and conferences, grant administration and monitoring, provision of technical assistance and preparing regular correspondence to other SOC staff.
- Conduct community system development, resource mapping, outreach and evaluation activities.
- Work with the System of Care Program Manager to implement SOC as requested.
- Actively solicit meaningful parent/family involvement in this project in coordination with team members.
- Plan and implement conferences, trainings and in-service activities with SOC team members and other community partners.
- Liaison:
- Develop and maintain contacts with appropriate Central Susquehanna Intermediate Unit staff, other service agencies and other related Central Susquehanna Intermediate Unit programs.
- Work closely with regional and state Department of Health staff and other regional service providers to coordinate the development of services intended to fill identified gaps in services.
- Work collaboratively with the Elks Nurse staff to assure a full continuum of care is offered to the children and families served.
- Assist the Pennsylvania Chapter of the American Academy of Pediatrics (PAAAP) with continuing development of the Medical Home Initiative within the region.
- Attend Pennsylvania Departments of Education, Health and Public Welfare-sponsored meetings and workshops regarding project implementation.
- Participate in Center for Schools and Communities staff meetings and participate in special meetings and/or task force meetings.
- Attend specified conferences and trainings to remain current in the field.
- Data Collection/Evaluation:
- Assist in developing appropriate project management, evaluation methods and data collection instruments for each component of the initiative to demonstrate effectiveness.
- Assist with training community partners and other stakeholders on use of instruments.
- Assist with data collection and processing.
- Assist with preparing reports.
- Information Processing:
- Review and distribute information pertinent to project operations.
- Assist in the preparation of monthly, quarterly and annual reports on project activities.
- Research and prepare correspondence and informal bulletins for local and statewide programs.
- Inter-project Coordination:
- Assist in coordinating Center projects to facilitate synergies in resource sharing, professional development and service delivery.
NON-ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Other duties as assigned.
Education, Experience and Skills Required:
- Minimum requirements are a Bachelor's Degree in education or one of the Social sciences and at least 5 years experience in a closely related field.
- Excellent verbal and written communication skills.
- Experience in human service, education and health systems.
- Understanding of group dynamics and its impact on organizational change.
- Ability to prioritize and organize multiple activities.
- Ability to work independently and willingness to be a team player.
- Ability to develop and maintain positive relationships with colleagues and clients both internal and external.
- Experience in program and community systems planning, development, education, and collaboration.
- Experience with work involving children, adolescents and families; experience with children with special health care needs and their families desired.
- Experience working with individuals of diverse cultures and ethnicities.
- Ability to work with and manipulate census and research data.
- Intermediate computer literacy including: word processing, database entry and manipulation, and internet searches. Experience with Power Point, FilemakerPro, Microsoft Publisher, and Microsoft Project is an advantage.
- Public speaking experience.
- Group facilitation/training skills.
- Five (5) years field experience.
- Valid driver’s license.
- Current Act 151 (Child Abuse) and Act 34 (Criminal Record) and Act 114 (FBI Fingerprint) clearances are required.
Physical Requirements:
- Lifting and moving materials for meetings/conferences.
- Visiting various program sites.
- Public speaking.
- Word processing.
- Driving for statewide travel.
Time and Site Requirements:
- Twelve month position, 260 day position.
- Attendance at work is a requirement of the position; any and all absences must be approved by the immediate supervisor.
- Frequent overnight travel for trainings, meetings and conferences.
- Ability to work from home.
- Ability to manage and maintain a flexible work schedule to accommodate frequent evening community meetings.
