FY 2005-06 Cross Systems Collaborative Infrastructure (Family Service System Reform -FSSR) Grant Semi-Annual Management Report

Overview

The Semi-Annual Management Report is designed to have collaborative partnerships assess their progress as they work to build an effective continuous learning environment using the goals, strategies and actions identified in the FY 2005-06 grant application (submitted in April 2005).  Information requested for this management report is for the period of July 1, 2005 – December 31, 2005.  Submission of this report is required for all collaborative partnerships receiving FSSR grant funds and is due on or before January 15, 2006.  Failure to submit this report may result in the Office of Children, Youth and Families (OCYF) delaying payment of grant funds. 

Report Submission

Grantees can submit the semi-annual report using traditional mail or electronically via e-mail.
